  1. Official transcripts from EACH university, college or other post-secondary institution that the applicant is currently enrolled in, or has previously attended. This includes transcripts for all Letter of Permission and exchange courses, as well as study abroad programs.
    1. Ontario university transcripts should be requested through OUAC Transcript Request section in the online application. Western University transcripts do not need to be requested through OUAC.
    2. Ontario transcripts that cannot be requested through the OUAC application and/or transcripts from a post-secondary institution outside of Ontario but within North America, including Letter of Permission and exchange courses: official transcripts must be sent directly from the institution(s) to you. Once Western has received your application from the OUAC, you will be prompted to activate your Western Student Center, where you will be required to scan and upload these transcripts.
    3. International education, including Letter of Permission, exchange courses or prerequisite courses: coursework must be assessed through World Education Services (WES). WES evaluations can take several weeks to process, so applicants are advised to request a WES evaluation several weeks in advance of the application deadline. Translation is required for prerequisite course synopses, if not originally in English. You must select:
      Western University as the recipient
Undergraduate Admissions as the school/division, and
WES ICAP - Course-by-Course evaluation to be sent by electronic delivery.

Documents must be submitted in the format(s) specifically requested on your Western Student Center. Hard copy documents will not be accepted by Western University or the Schulich Medicine & Dentistry Admissions Office.

    Important Notes

It is the applicant's responsibility to ensure all documentation is submitted to the appropriate office by deadlines indicated.

Falsification or misrepresentation of information from any portion of the application process, including interviews, will result in removal of the application from consideration and may be considered in any future applications to the University.
